目的 探讨磁共振弥散加权成像(magnetic resonance-diffusion weighted imaging,MR-DWI)联合超声造影在前列腺认知融合靶向穿刺的诊断价值。方法 收集2021年1月至2022年12月在绍兴市中心医院行经直肠前列腺穿刺的140例患者的临床资料,根据穿刺影像辅助方式不同将其分为A组(MR-DWI联合超声造影认知融合靶向穿刺,23例)、B组(MR-DWI联合超声认知融合穿刺,37例)、C组(超声造影穿刺,30例)和D组(普通系统穿刺,50例),并按照分组进行个体化穿刺活检,比较各组穿刺前列腺癌病理阳性率。结果 140例患者中病理诊断阳性54例,阴性86例。A组阳性率65.2%(15/23),B组阳性率37.8%(14/37),C组阳性率36.7%(11/30),D组阳性率28.0%(14/50),A组患者的前列腺癌检出率高于B组、C组及D组(P<0.05)。结论 MR-DWI联合超声造影前列腺认知融合靶向穿刺可提高前列腺癌的检出率,具有临床诊断意义。
Objective To investigate the diagnostic value of magnetic resonance-diffusion weighted imaging (MR-DWI) combined with contrast-enhanced ultrasound in prostate cognitive fusion targeted puncture. Methods The clinical data of 140 patients who underwent transrectal prostate puncture in Shaoxing Central Hospital from January 2021 to December 2022 were collected. They were divided into group A (MR-DWI combined with contrast-enhanced ultrasound cognitive fusion puncture, 23 cases), group B (MR-DWI combined with ultrasound cognitive fusion puncture, 37 cases), group C (contrast-enhanced ultrasound puncture, 30 cases) and group D (common system puncture, 50 cases) according to different auxiliary methods of puncture images. The pathologic positive rate of prostate cancer in each group was compared by individual puncture biopsy. Results Among the 140 patients, 54 were positive and 86 were negative. The positive rate of group A was 65.2% (15/23), the positive rate of group B was 37.8% (14/37), the positive rate of group C was 36.7% (11/30) and the positive rate of group D was 28.0% (14/50). The detection rate of prostate cancer in group A was higher than that in groups B, C and D (P<0.05). Conclusion MR-DWI combined with contrast-enhanced ultrasound for prostate cognitive fusion targeted puncture can improve the detection rate of prostate cancer and has clinical diagnostic significance.
