替加环素导致凝血系统不良反应的危险因素及可能机制研究进展

替加环素是一种具有广泛抗菌活性的甘氨酰环素类抗菌药物,因其耐药率低、抗菌活性强,临床使用量逐年增加。近年来,伴随着使用量的增加其安全性争议也越来越多,关于凝血系统的不良反应报道明显增多,研究报道相关危险因素也不尽相同。因此,本文综述了替加环素引起的凝血系统不良反应及其危险因素。替加环素用药期间可出现纤维蛋白原降低、活化部分凝血活酶时间、凝血酶原时间和凝血酶时间延长等凝血参数的改变,长疗程使用替加环素、用药前低纤维蛋白原值、腹部感染、高龄和肾功能损伤可能是替加环素相关凝血异常的危险因素。

Tigecycline(TGC) is a glycylcyclines antibacterial drug with a wide range of antibacterial activities. Due to its low drug resistance rate and strong antibacterial activity, the clinical usage of TGC increased by years. In recent years, with the augment of TGC use, security controversies are also growing, especially the reports of coagulation system adverse reactions induced by TGC have significantly increased, while the risk factors reported are different. Therefore,this article reviewed the coagulation system adverse reactions induced by TGC and its risk factors. Changes in coagulation parameters such as decreased FIB, prolonged APTT, PT and TT may occur during tigecycline treatment, and long-term use, low FIB values before treatment, abdominal infection, age and renal impairment may be the risk factors for tigecycline-related coagulopathy.
