Objective To observe the value of 18F-prostate specific membrane antigen (PSMA)-1007 PET/CT for differentiating normal ganglia and lymph node metastasis of prostate cancer. Methods Data of 80 patients with prostate cancer who underwent early and delayed 18F-PSMA-1007 PET/CT examinations were retrospectively analyzed. The display rates of stellate ganglion, celiac ganglion and sacral ganglion were observed, and the maximum standard uptake value (SUVmax) and ΔSUVmax (SUVmax in delayed phase-SUVmax in early phase) were compared between ganglia and systemic metastatic lymph nodes, hence the differential value of PET/CT was analyzed. Results Among 80 prostate cancer patients, PET/CT showed at least 1 ganglion (stellate/celiac/sacral) in 60 patients (75.00%, 60/80). The display rate of PET/CT of celiac ganglion, stellate ganglion and sacral ganglion was 66.25% (53/80), 56.25% (45/80) and 30.00% (24/80), respectively, with significant differences(χ2=22.44, P<0.001). SUVmax in early and delayed phase and ΔSUVmax of stellate/celiac/sacral ganglion were all significantly lower than those of metastatic lymph nodes (all P<0.001), and there was significant difference of the constituent ratio of ΔSUVmax between ganglion and lymph node (P<0.001). SUVmax of ganglions were rather stable, while of lymph node in delayed phase was higher than in early phase. Conclusion According to anatomical location and SUVmax, 18F-PSMA-1007 PET/CT was helpful for differentiating normal ganglia and lymph node metastasis of prostate cancer.
