目的 探讨CT导向下科研所消融术治疗腰椎间盘突出的价值。方法150例腰椎间盘突出于CT导向下行臭氧消融治疗。突出物为旁中央型者经神经根与硬膜囊之间穿刺突出物达盘内,突出物为外侧型者经神经根外侧,小关节突内缘穿刺突出物达盘内,分别于盘内及突出物中央注射浓度为60μg/ml臭氧5-15ml。结果流向臭氧后CT扫描显示臭氧在间盘内呈裂隙状、髓核腔内积聚状、弥散分布状。臭氧注射后神经根轮廓全部显露、“淹没征”消失者占29%;神经根大部分显露、“淹没征”部分消失者占50%;神经根显露不明显者占21%。治疗后3个月随访显效、有效和无效分别占82%、13%和5%。结论CT导向下臭氧消融术穿刺准确,可监测臭氧分布及注射后改变,疗效满意。

Objective To explore the value of CT guided O3 injection in the treatment of lumbar disc herniation. Methods All 150 patients with herniation of lumbar disc were treated with O3 injection under the guidance of CT. The puncture rout for midline or para-midline herniated nucleus pulposus (HNP) passed between the nerve root and dural sac to disk, and that for posterolateral HNP between the nerve root and the inner margin of articular facet to disk. A total of 5-15 ml of ozone with concentration 60 μg/ml were injected both to disk and the center of HNP. Results After injection of ozone, the ozone were distributed as localized shape, cranny shape and dispersion shape. After the treatment, 29% HNP the obliteration of nerve root vanished, 50% partially and 21% nerve root remained obliterating. The excellent, good and poor clinical efficacy was 82%, 13% and 5%, respectively 3 months after treatment. Conclusion CT guided O3 injection is an accurate, safe, and effective method in the treatment of lumbar disc herniation.
