冠心病脉图特征与冠脉狭窄程度、超声心动图的关联研究

目的:探讨冠心病脉图特征与其病理变化的关系,寻找与脉图参数可能相关的病理生理指标。方法:(1)运用脉象检测系统采集冠心病患者的脉图,采用时域方法提取患者的脉图参数;并采用Megas多谱勒彩色超声仪检测其心脏超声心动图指标;按Judkin方法进行造影,检测冠脉狭窄程度。(2)采用典型相关分析方法等分析冠心病患者脉图参数和病理变化的相关性。结果:①冠心病患者冠状动脉狭窄程度与脉图参数h3/h1、w1/t、h3、t5/t4呈正相关;尤其w1/t与冠脉狭窄程度最密切。②心脏超声指标中主A瓣口径、二尖瓣E峰的变化与脉图h1、h5、h5/h1最密切相关,且呈负相关。结论:冠心病患者脉图参数与冠脉狭窄程度、超声心动图指标存在一定的相关性,表明冠状动脉及心脏形态的病理改变可能是冠心病患脉图特征变化的病理基础之一,也为脉诊无创检测手段提供了理论支持。

This study is designed to explore the relationship between pulse graph traits and pathological information of coronary heart disease (CHD) patients, in an attempt to find out both pathological and physiological indexes that are associated with pulse graph parameters. In this study, 1) CHDpatients' pulse graph were collected using the pulse detection system, and the pulse traits analyzed through the time domain method. CHD patients' ultrasound cardiograph were collected using Megas Doppler multicolor ultrasound instrument, and the coronary angiography (CAG) was conducted through Judkin method. 2) We analyzed the relationship between time domain parameters and pathological information, using analytical method of canonical correlation. Study results showthat 1) there is a positive correlation between magnitude of coronary artery stenosis and such pulse graph parameters as h3/h1,w1/t,h3,t5/t4. w1/t is, in particular, closely associated with the magnitude of coronary artery stenosis. Aclose relationship is also seen between h1,h5,h5/h1 and Doppler ultrasonic index of heart; 2) They are negative correlation. In the context of blood- fat ingredients, a change in the lowdensity lipoprotein (LDL) is closely associated with h1,h5 and h5/h1 of pulse graph in CHD.We believe there are certain correlations between pulse graph parameters and coronary artery stenosis and ultrasound cardiograph. Study results indicate that pathological changes of coronary artery and heart could be a material base to shape up pulse graph traits in CHDpatients. In addition, Pulse Diagnosis featured with non- invasive detection provides a theoretical support.
