目的:观察培土生金法对慢性阻塞性肺疾病(CORD)患者多因素分级系统(BODE)指数及生活质量的影响.方法:选择COPD稳定期患者60例,随机分成治疗组和对照组.对照组按需加用支气管舒张剂或糖皮质激素治疗,治疗组在对照组基础上加用培土生金法治疗,计算BODE指数及其各项目的评分,包括体重指数(BMI)、气流阻塞程度(FEV1% pred)、呼吸困难(MMRC)、运动能力(6MWD).采用St·George S呼吸问卷(SGRQ)评价患者的生活质量.观察治疗前后BODE指数及SGRQ评分的变化.结果:60例入选病例,研究结束时共有3例病人失访,合格病例57例.①治疗组BMI,FEV1,6MWT,MMRC及BODE指数积分治疗后较治疗前有所降低(P<0.05或P<0.01).对照组BMI,FEV1,6MWT,BODE指数积分治疗后无明显变化,MMRC积分治疗后较治疗前有所降低(P<0.05),治疗后治疗组与对照组BODE指数比较有显著差异(P<0.01).②SGRQ总分,两组治疗后SGRQ评分(56.98±14.14),(64.86±8.45)分均较治疗前(73.64±11.29),(73.68±8.84)分下降,两组治疗后比较有显著差异(P<0.01).结论:培土生金法能显著改善慢性阻塞性肺疾病患者稳定期BODE指数及生活质量.
Objective:To observe the effect of strengthening earth to generate metal method on BODE index and quality of life in chronic obstructive pulmonary disease(COPD) patients.Method:Sixty patients with stable COPD were randomly divided into treatment and control group.The control group was treated with bronchodilator or glucocorticoid,the treatment group was treated with strengthening earth to generate metal method on the basis of rrgular treatment in the control group,and the calculation of the BODE index score included body mass index(BMI),degree of airflow obstruction(FEV1% pred),dyspnea(MMRC),exercise capacity(6MWD).The St·George S Respiratory Questionnaire(SGRQ) was used to evaluate the quality of life of patients.BODE index and the SGRQ score changes before and after treatment were observed.Result: In selected 60 cases,on the end of the study a total of 3 patients was lost,57 patients entered qualified patients.① In the treatment group BMI,FEV1,6MWT,MMRC and the BODE index were decreased after treatment compared with before treatment(P<0.05 or P<0.01).In control group,BMI,FEV1,6MWT,BODE index after treatment had no significant change,MMRC decreased after treatment comparted with before treatment(P<0.05),compared with the control group,the BODE index in treatment group was significantly different(P<0.01).②SGRQ total score and SGRQ total score after treatment were decreased(73.64±11.29,56.98±14.14;73.68±8.84,64.86±8.45),two groups showed significantly different(P<0.01).Conclusion: The strengthening earth to generate metal method can significantly improve stable BODE index and quality of life in patients with chronic obstructive pulmonary disease.
