目的 探讨超声引导下液体隔离法的实施方法、技术要领及其在颈部结节状病变热消融治疗中的应用价值。方法 对1126例罹患颈部结节状病灶患者行超声引导下经皮射频或微波消融治疗,消融时于超声引导下向病灶与其周围结构的潜在间隙内注射隔离液,制作隔离带。重点研究液体隔离法在提升穿刺路径安全性、降低正常结构热损伤、提高病灶消融彻底性等方面的作用。结果 在甲状腺、甲状旁腺、颌下腺、淋巴结周围等14个部位成功实施了液体隔离法,总结出液体隔离法的技术要领和超声影像特点,证实采用液体隔离法前后颈部消融热损伤发生率(15% vs. 0.7%,χ2=53.18, P<0.01)以及病灶一次消融残余率(7.4% vs. 0.29%,χ2=34.32, P<0.01)的显著降低具有统计学意义。结论 研究结果表明超声引导下的液体隔离法实施过程较为简洁,隔离效果确切,可重复性好,是颈部结节状病变热消融治疗时极为有效的安全措施。

Objective We aim to design and conduct hydro-dissection for improving the safety and curative effects of ultrasound-guided percutaneous thermal ablation therapy of neck nodular lesions. Methods Percutaneous thermal therapy by using radiofrequency ablation or microwave ablation was carried out on 1126 patients suffering neck nodular diseases. As a part of the treatment procedure, ultrasound-guided injection of separating liquid into between the target nodules and surrounding structures to form a separating zone was performed prior to starting ablation. The focus of study was whether the hydro-dissection technique had improved the safety of puncture route, reduced the thermal injury to normal structures, and enhanced the curative effects of various neck nodules. Results Hydro-dissection technique had been successfully achieved in as many as fourteen parts related to thyroids, parathyroids, submandibular glands and lymph nodes. The ways of technical operations and ultrasonic features of this maneuver were well established and understood. The advantages of this maneuver was faithfully verified by the statistically significant reduction of undesirable heat damage to lesion-adjacent structures(15% without hydro-dissection down to 0.7% with hydro-dissection , P<0.01) and decrease in incomplete ablation of the lesions(7.4% without hydro-dissection down to 0.29% with hydro-dissection, P<0.01). Conclusion Proved successfully in practice, hydro-dissection technique under ultrasound-guidance is virtually simple to design, easy to control, likely to repeat and reliable to function. Application of this technique is supplementary to the safety measures involved in thermal ablation of various neck nodular lesions.
