目的探讨生物型加长柄人工股骨头置换治疗老年股骨粗隆间骨折的临床疗效。方法收集笔者单位2011年3月-2014年3月应用生物型加长柄人工股骨头置换治疗的32例老年性股骨粗隆间骨折患者资料,其中男性14例,女性18例;年龄75~89岁,平均81.6岁。骨折按改良的Evans-Jensen分型,Ⅲ型24例,Ⅳ型6例,Ⅴ型2例。评价随访期间患者骨折愈合情况及髋关节功能。结果 32例获随访10~24个月,平均15.6个月,随访期间无髋内翻、无假体柄松动移位及折断、无脱位及骨不愈合等失败病例。髋关节功能按Harris评分评价:优22例,良8例,可2例,优良率93.75%。结论对于老年性股骨粗隆间骨折,加长柄人工股骨头置换术是一种安全有效的治疗手段。
Objective To evaluate the clinical efficacy of biological type long handle artificial femoral head replacement in elderly intertrochanteric fractures. Methods From Mar.2011 to Mar.2014,32 cases of elderly intertrochanteric fractures were treated by biological type long handle artificial femoral head replacement, including 14 males and 18 females;with the average age of 81.6 years(ranging from 75 years to 89 years).According to improved classification of Evans-Jensen,there were 24 cases with type Ⅲ fracture and 6 with type Ⅳ and 2 with type Ⅴ.Fracture healing and hip joint function during the follow-up were evaluated. Results Thirty-two cases were followed up for 10 months to 24 months(15.6 months on average ).During the follow-up period, no hip varus, no loose prosthesis handle displacement or breakage, no dislocation or failure such as nonunion occurred.The function of the hip was graded by the system of Harris and the excellent and good rate was 93.75%(excellent in 22 cases, good in 8 cases, and fair in 2 cases). Conclusion The long handle artificial femoral head replacement is safe and effective for treating elderly patients with intertrochanteric fractures.
